Iraq & Washington's Systemic Failure

On the fourth anniversary of the disastrous Iraq War, it's tempting to lay the blame at the doorstep of George W. Bush and Dick Cheney, since they surely deserve a lot of it. But the war also marked a systemic failure of the Washington political/media hierarchy, a perfect storm of colliding elements -- aggressive Republicans, triangulating Democrats, careerist journalists and bullying pundits.
